Forum Health Provider is moving to a new electronic health records system called Power2Patient.

This new system includes a patient portal which will become the new hub for interacting with the clinical staff. On the patient portal you can:

Review your treatment plans
Review your medications and supplements
Fill out consent and intake forms
Review upcoming appointments
Request new appointments (you can still call us as well)
Upload and Download documents and lab reports
Review and reprint your invoices
Direct message the clinical team (a secure system that keeps messages attached to your chart)

We think this new system will greatly improve our ability to serve you and give you much better access to the information that you need. We will be asking all patients to begin using the patient portal as soon as possible.
